Polk County residents have more than 50 Medicare Advantage plans to choose from for 2027, and many carry a $0 monthly premium. That sounds like good news, and it is. But when every plan costs "nothing," the real differences hide in the details: your doctors, your prescriptions, and how much you could pay out of pocket in a bad year.
Plans change every year. A plan that fit you perfectly in 2026 may have a new drug list, a different network, or a higher out-of-pocket limit in 2027. The Annual Enrollment Period, October 15 through December 7, is your once-a-year chance to make sure your coverage still fits.
What's available in Polk County for 2027
In the 33860 ZIP code (Lakeland area), 51 Medicare plans are available for 2027 through the five organizations I represent. That includes 20 Medicare Advantage plans with drug coverage, 7 Medicare Advantage plans without drug coverage, plus Medicare Supplement, dental, vision and hospital indemnity options.
Here is a sample of the $0-premium Medicare Advantage plans from Aetna, Humana, UnitedHealthcare, BayCare and Devoted, sorted by out-of-pocket maximum:
| Plan | Type | Monthly premium | Medical deductible | Out-of-pocket max |
|---|---|---|---|---|
| CareOne Plus | HMO-POS | $0 | $0 | $1,250 |
| Humana Gold Plus H1036-230 | HMO | $0 | $0 | $2,400 |
| CareFree Platinum Giveback | HMO | $0 | $0 | $3,000 |
| Humana Gold Plus Giveback H1036-266 | HMO | $0 | $0 | $3,400 |
| AARP Medicare Advantage from UHC FL-003P | HMO-POS | $0 | $0 | $3,500 |
| AARP Medicare Advantage from UHC FL-0006 | HMO-POS | $0 | $0 | $3,800 |
| Aetna Medicare Select H1609-087 | HMO | $0 | $0 | $3,900 |
| Devoted Core 065 FL | HMO | $0 | $0 | $3,900 |
| CareAccess | HMO | $0 | $0 | $4,000 |
| Humana Value Choice | PPO | $0 | $0 | $5,600 |
Every plan above includes dental, vision and hearing benefits. Notice the spread: the out-of-pocket maximum ranges from $1,250 to $5,600, even though every premium is $0. Plan details as of October 2026; benefits vary by plan and are subject to change.
Look past the $0 premium
When most plans cost the same each month, these are the numbers that separate them:
- Out-of-pocket maximum. This is the most you would pay for covered medical care in a year. A $1,250 limit versus a $5,600 limit is a $4,350 difference if you have a hospital stay or a major surgery.
- Drug costs. Each plan has its own formulary and pharmacy pricing. The same prescriptions can cost a few hundred dollars a year on one plan and well over a thousand on another.
- HMO vs. PPO. Most Polk County options are HMOs, which use a set network and usually need referrals. A PPO lets you see out-of-network doctors, typically at a higher cost.
- Giveback plans. Some plans, labeled "Giveback," reduce your Part B premium. That can put money back in your Social Security check each month.
- Extras. Dental, vision and hearing are common, but the dollar amounts and coverage levels vary a lot from plan to plan.
How to choose the right plan
The best plan is the one that fits your doctors, your prescriptions and your budget. Before AEP, gather three things:
- Your doctors. List every doctor and specialist you see, including the hospital you prefer (Lakeland Regional, BayCare, AdventHealth or others). Confirm each one is in the plan's 2027 network.
- Your prescriptions. Write down every medication with the dose. Total drug cost, not just the copay, is often the biggest difference between plans.
- Your budget for a bad year. Ask yourself: if I needed surgery next year, could I comfortably pay the out-of-pocket maximum? If not, a lower limit may matter more than any extra benefit.
With those in hand, a side-by-side comparison takes minutes instead of hours.
